WAIVER AND RELEASE OF LIABILITY -- READ BEFORE SIGNING In consideration of being allowed to participate in any way in the Project Fit Program, its related events and activities, I, the undersigned, acknowledge, appreciate, and agree that: The risk of injury from the activities involved in this program is significant, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury or death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my child's participation. I willingly agree for my child to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my child's presence or participation, I will remove my child from participation and bring such to the attention of the Company immediately. I understand this Fleet Feet Sports location is an independent licensee of Fleet Feet, Incorporated and independently operates this Project Fit program. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Fleet Feet Sports Wilmington (name of business entity) d/b/a FLEET FEET SPORTS, their agents, employees, coaches, volunteers, officers, directors, franchisors, successors and assigns, the City of Wilmington, NC, and any and all sponsors, their representatives and successors ("Releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property associated with my child's presence or participation, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law. I attest that my child is in good health and physically capable of participating in the Project Fit Program, and my child's medical care provider has approved my participation. Further, I hereby release, consent to, and authorize, in advance, any such use of my child's name, photograph, voice or likeness by the foregoing parties in any manner they deem appropriate and necessary without remuneration to me or my child. I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS.

Here are the general rules for this club: -No horseplay and keep your hands to yourself. We don’t want anyone injured. -Be respectful to others. No teasing and listen to your coaches. -Arrive on time, wear comfortable shoes and clothing, and bring a snack and water. -Stay with your group -Be positive -If you don’t feel well or start to feel an injury, tell an adult. -Never leave your group without telling your coach. We need to keep track of you! -Parents are responsible for picking their child up at the end of each session So how do we start? 1. Each time, we take roll. 2. Next we will do warm up /active exercises. 3. Then we will do running / walking workouts. 4. Lastly we will do cool down stretches. It is very important to warm up and stretch at cool down so your muscles don’t get injured. Now let’s go have fun! NOTE: Children should bring water to every workout. It is also recommended students bring a healthy snack to nibble before or after workouts.
